90-DAY DIABETES PREVENTION OUTLINE FOR INDIVIDUALS
Goal: At the conclusion of coaching sessions, participants are equipped to establish new habits and routines to reverse a prediabetes diagnosis
Sessions 1-3: Focus on prediabetes education and creating health and wellness goals
1. Explain in layman’s terms what prediabetes and Type 2 diabetes are, the causes and consequences of these diagnoses, create 3-month goals to include activity and eating shifts.
2. Review of food diary, introduction to SMART goals; create SMART goals for the next two weeks.
3. Reading labels and understanding what to look for, identifying systems for tracking food and activity, review food diary.
Sessions 4-5: Review and mastery of SMART goals, intentional eating and shopping, managing thoughts
4. Physical activity tracking: what is working, overcoming barriers and obstacles, how to master your goals.
5. Your brain programming and habits, shifting mindset to support wellness goals, mindful eating, dealing with food pushers.
Sessions 6-7: Maintaining lifestyle changes
6. When weight loss stalls, accountability, increasing awareness of thoughts that lead to success and barriers.
7. Celebrating wins, revisiting food targets, strengthening food and activity plans, getting support, maintaining momentum
